A penalty is being taken and the defending team crosses the line before the ball is struck. What follows?

The penalty is re-taken. The side facing the hit erred, so the penalty is taken again.

A penalty is being taken and a defender rides out early and no goal results. What follows?

The penalty is re-taken. Infringement by the team facing the hit: re-take it.

A penalty is being taken and the hitting side has a player in front of the ball. What follows?

A Penalty 5a to the other side, from where the penalty was taken. The side taking the hit erred, so the other side gets a Penalty 5a.

A penalty is being taken and the player taking the hit strikes the ball twice. What follows?

A Penalty 5a to the other side, from where the penalty was taken. Infringement by the team fouled: Penalty 5a against them.

A penalty is being taken and both sides get the procedure wrong. What follows?

The penalty is re-taken. When both err, the penalty is re-taken at the same point under the same conditions.
